Documentation requirements for health insurance claims

With each health insurance claim (reimbursement request), you are required to submit supporting documentation verifying your coverage and premium payment.

Health Insurance Documentation Requirements

Documentation for health insurance premium reimbursement claims needs to include these five pieces of information:
  1. Insurance Provider
  2. Type of Health Insurance
  3. Name of Primary Policyholder
  4. Period of Coverage for Amount Claimed
  5. Proof of Payment for Amount Claimed

What Types of Documents Include this Info?

Documentation must show all five items. For example, you could provide a billing statement from an insurance provider with all information on one document (this is recommended), or you may combine multiple documents that together show all five items. 

Example: Health Insurance Premium Reimbursement Documentation

In this example, the Participant submitted a billing statement from their insurance company that lists all 5 items on single statement.
